For established Allied Dermatology patients, login to the app using your Patient Portal User Name and Password. If you are unsure of your sign on, please call the office at 866-337-6631 or click on the Forgot Password link on the app.
- Click request e-visit
- Select the Provider for the e-visit
- Answer all questions presented to the best of your ability
- After completion of the questions, hit the submit key
Your Provider will be notified and correspond with you appropriately, either through the app or by telephone. Please be available for any further questions the Provider may have regarding your condition.
