Medical Records & Instructions
Medical records can be accessed through your patient portal at any time. If you are unsure of your portal information please contact the office and any staff member can further assist.
Below you will find the HIPAA Compliance Requirements needed in order to properly process each records request.
HIPAA Compliance Requirements
- Patient information: Full name, date of birth
- Description of information to be disclosed: Specific medical records or details to be released, including diagnoses, treatment plans, lab results, etc.
- Recipient of information: Name and contact details including fax number of the person or entity authorized to receive the medical records.
- Expiration date: A clear timeframe for when the authorization expires or 90 days.
- Signature and date: Electronic or Patient’s signature, or authorized representative’s signature if applicable, along with the date the form was signed.
Medical Records must be requested in a written or electronic format. You may fill out a medical request form at any of our office locations. Requests can also be submitted through either the Patient Portal or our website. Follow the link to fill out our medical request form. Once filled out it can be uploaded and submitted to the records department. When submitting your form please note the reason why the information is being released. Such as for treatment, payment, healthcare operations or research.
Fees
As a courtesy we will electronically fax medical records to collaborating providers or for transfer of care. All printed medical record requests will incur a fee.
